We have set up a FERPA-compliant system for you to submit electronic forms, rather than emailing them.  Please submit all electronic forms and any applicable supporting information by following the instructions below.

  1. Download PDF it to your computer.  Complete the form using Adobe Acrobat Reader (free download; don’t download any other offers) 
  2. Save completed form again before uploading into the tinyurl.
  3. If a form requires your signature, please sign it with the Adobe Acrobat Reader signing tool.  Please note special instructions below for UR/IS (undergrad research or independent study) forms.

  5. We will secure any additional signatures and forward to the CALS Academic Dean’s office.

UR/IS (Undergrad Research/Independent Study) forms:

Independent Study: An Independent Study is typically a continuation in greater depth of a topic covered in a regular course, and allows students to study topics of particular individual interest. Specialized instruction related to design teams are most often independent studies. 

Undergraduate Research: Students can earn Undergraduate Research credit for research projects they carry out under faculty supervision. The research typically creates an artifact such as a paper, presentation, or technical product.

 APSC Departmental forms:

Restricted Elective Contracts (REC)

  • This form is required to complete a graduation analysis, which is required before you can apply for your degree.  Students who have completed at least 78 credits towards the APSC degree need to complete the graduation analysis this semester.

  • Use Adobe Acrobat Reader to complete the REC for the option and emphasis you are following and sign it using the Adobe Acrobat Reader signing tool. 

  • Share with your advisor using google share. 

  • You must meet with your advisor and obtain their signature before submitting the REC to the secure site.

  • We will contact you to schedule the graduation analysis
